Overview

Faculty, staff, and students at the University of Arkansas have access to a suite of tools for creating and managing forms. These tools can be used to:

• RSVP for events and manage event logistics
• Gather feedback or contact information
• Conduct research and assessments
• Request services or access
• Collect data for reporting and workflows

This guide outlines what form software is available, who can access it, and where to find resources and training to get started.

Microsoft Forms

• Access: All @uark.edu accounts via myapps.microsoft.com. Look for the Forms icon.
• Training: Microsoft Forms Help & Learning
• Support: Microsoft 365 Support through UARK IT Services

TeamDynamix Service Forms

• Access: Requires onboarding as a technician. Request access via TeamDynamix Admin team.
• Includes: Defining services, creating request forms, and technician training.
• Training: Technician Training Knowledge Base
• Support: UARK TeamDynamix Support

Blackboard Surveys

• Access: Faculty automatically have access. Staff must request a course shell.
• Training: Blackboard Help or tips.uark.edu
• Support: UARK Blackboard Support

Qualtrics

• Access: Request via uark.qualtrics.com or via myapps.microsoft.com
