PWD: How to Request Release of Educational Records

Overview

PWD students, especially high school students, may want to release their educational records to their parents, legal guardians, or some other trusted third-party. FERPA protects such records unless a student requests that others should have access. 

Step 1

Navigate to training.uark.edu and click the My Profile link:

Step 2

Click the Login button:

Provide your email address and password. Then, click the Login: button:

Step 4

You'll now be viewing your profile. Scroll down a bit to locate the Request Forms link. It's directly above your list of active courses. Click the link:

Step 5

Locate the course for which you would like to release information and click its Add Release button:

Step 6

Fill out the required fields of the Release of Course Information Consent form. Be sure to check the Acknowledgement check box, near the bottom. Then, click the Submit button:
